Field Trip: Sweet Petes - Candy Maker

Time: 10:00am-2:30pm

Cost: $20.00

I hereby grant permission for my campers to be photographed and/or recorded during camp activities and field trips organized by Cornerstone of Jacksonville. 

I understand that these photographs, video recordings, or other media may be used by the camp for purposes including, but not limited to, promotional materials, social media, website content, newsletters, and other publication by Cornerstone of Jacksonville, the Kids Hope Alliance, and/or the City of Jacksonville. 

I acknowledge that: No compensation will be provided for the use of these images or recordings. All images and recordings will remain the property of Cornerstone of Jacksonville. I release and hold harmless Cornerstone of Jacksonville, the Kids Hope Alliance, the City of Jacksonville, and their staff and representatives from any claims arising out of the use of such media.

As the parent/legal guardian of the student listed above, I grant permission for him/her to participate in this field trip. Furthermore, I give my permission for my child to receive medical treatment in the event of an emergency at my cost. In consideration for my child’s attendance on this field trip, I/we waive and release Cornerstone of Jacksonville, Inc., Kids Hope Alliance, The City of Jacksonville, and/or it’s agents, employees, Board of Directors, and/or assigns from all claims, demands, causes of action, damages or suits at law and equity of whatsoever kind, including but not limited to claims for personal injury, property damage, medical expenses, loss of services, on account of or in any way related to or growing out of my child’s attendance, transportation, and/or participation in this field trip. This waiver and release of liability is intended to and does release all parties listed above, from any and all liability for damages or injuries on account of or in any way related to or growing out of my negligence, my child’s negligence, the negligence of another student, the negligence of a school employee’s and/or the negligence of a third party, including but not limited to negligence in the, maintenance and upkeep of the transportation vehicle and equipment, negligence in training and/or negligence in supervision in addition to but not limited to any other negligence of all parties listed above, and/or their agents, related companies, servants, employees, insurers, Board of Directors, successors and assigns not mentioned. I/we understand that all parties mentioned above, and/or their agents, related companies, servants, employees, insurers, successors and assigns are not responsible for any lost or stolen valuables while on this field trip. I agree any violation of the Agreement including inappropriate behavior by myself or my child, as determined by Cornerstone, may result in loss of the ability for my child to attend this field trip. If any provision of this agreement is deemed unenforceable or void, the remainder of this agreement shall not be affected and will be fully enforceable. I enter into this agreement knowingly and voluntarily. This field trip is NON-REFUNDABLE – NO EXCEPTIONS.

As parent/legal guardian of the camper listed above, I grant permission for him/her to participate in the Field Trip outlined above. I also give my permission for my child named above to receive medical treatment in the event of an emergency. I understand that I will be responsible for any expense related to any medical treatment they receive.
