Summer Camp Scholarship Information
Cornerstone of Jacksonville has partnered with the Kids Hope Alliance and the City of Jacksonville, to offer Summer Camp Scholarships, to eligible children, in Kindergarten thru 11th Grade, for Week #1 thru Week #6. The fee for eligible children that are granted a scholarship is $65.00, for the 6-week period (field trips are an additional charge). This $65.00 fee must be paid at the time of enrollment. Campers participating under the Summer Scholarship Program must attend a minimum of 4 days, each week, during the 6-week program. Parents must provide a copy of their child’s eligibility documentation to the camp office at the time of registration. Scholarships are awarded on a first come, first serve basis. Availability is limited.
Summer Camp Payment Policies
CHECK POLICY: Any checks written to Cornerstone of Jacksonville may be deposited electronically. If a check is returned for Non-Sufficient Funds, Cornerstone reserves the right to re-run/re-deposit said check, up to 8 times over the course of a year period in an attempt to collect funds. There will be a $40.00 (or the maximum Florida allows) fee assessed for any payments that are returned as a result of non-sufficient funds or a stop payment.
COLLECTIONS / CREDIT REPORTING: In the event your student’s account becomes past due, Cornerstone of Jacksonville, Inc. may report the outstanding balance owed to one or more credit bureaus via a collection agency. Outstanding balances will be reported under the student’s primary parent on file and/or the primary account contact on file. In addition, you will be responsible for any court cost, attorney fees, and or collection cost that may be added. Collection fees of up to 50% may be added for the recovery of your debt permitted by law, 15.U.S.C.1692f(1)[section 808(1)].
FEES - LATE PICK-UP: After 6:00pm a late pick-up fee of $10.00 per each 10-minute increment, per camper, including any portion of the increment will be due. The Cornerstone’s staff member on duty will be the official timekeeper for said billing purposes. All fees are due at the time of pick-up. If the fees are not paid at the time of pick-up, they will be billed to the primary parent on file.
FEES - LATE PAYMENT: Fees are due in full every Monday morning of each week, before the child starts his/her day. There will be a late fee of $25.00 if weekly payment is not received by Monday at noon (12:00pm) each week. Campers will not be permitted to stay on Tuesday unless payment for the week has been made in full. By signing this agreement, you acknowledge that you understand and agree to the weekly fees/rates stated within this application. Furthermore, you understand and agree that in the event of an outstanding past due balance on your child’s account, additional collection fees may apply. Campers with a past due balance for 2 or more days, may be removed from the summer camp program.
PAYMENT INFORMATION:
Fees are not pro-rated. If a child attends one day within a week, they are billed for the entire week.
No sibling discount is available.
All payments made are non-refundable. (Including Field Trips)
Fees are due in full every Monday morning of each week, before the child starts his/her day. Children may not be permitted to stay for camp unless his/her account is current.
The Parent/Legal Guardian who signs this agreement accepts full financial responsible for the child and agrees to pay any fees related to the child’s participation in the program.
All payments made to Cornerstone will be applied to the oldest invoice (based on the due date of each invoice). Cornerstone reserves the right to apply any payment to any outstanding invoices on the account, including but not limited to field trips, summer camp and late fees balance(s) regardless of the request.
Summer Camp General Policies
Authorization for Emergency Care: In case of accident or serious illness, and the program site is unable to reach me, I hereby authorize Cornerstone of Jacksonville to make whatever arrangements necessary to provide care and treatment for my child. In case of accident/serious illness where the immediate treatment of my child is not necessary, but he/she is unable to remain in the program, I will be contacted, or transportation arrangements will be made for my child. If the program is unable to reach me, I authorize Cornerstone of Jacksonville to contact one of the persons indicated on the enrollment form and ask them to pick up and transport my child home.
AUTODIALED CALLS, PRE-RECORDED MESSAGES, AND TEXT MESSAGES: By signing this agreement, you hereby consent to receive autodialed and/or pre-recorded telemarketing calls and/or Text Messages from or on behalf of Cornerstone of Jacksonville at the telephone numbers, including wireless numbers, provided to Cornerstone, unless otherwise indicated on page 2 or 3 of this agreement. In addition, by signing this agreement you consent to the use of these methods for collection purposes, if applicable.
FIELD TRIP PERMISSION / INFORMATION: Campers may participate in weekly field trips. Each field trip costs between $2.00 and $27.00 dollars in addition to the weekly rate. There will be one (1) to three (3) field trips each week. Campers are not required to attend every field trip. By signing this agreement, you give your child permission to participate in all field trips scheduled for the 2025 Summer Camp Program including transportation to Cornerstone’s Sports Complex (Fort Caroline Road). Furthermore, you agree to hold Cornerstone of Jacksonville, Inc. (DBA: Camp Discovery) and/or Cornerstone Christian School, Inc, and/or the Kids Hope Alliance, and/or the City of Jacksonville and/or any of their agents and/or affiliates, blameless of any/all liability resulting from injury sustained and/or loss of personal property on these field trips and/or on the campus grounds. Field Trip Sign-Ups are accepted on a first come, first serve basis.
Kids Hope Alliance Authorization: I give Kids Hope Alliance staff the authorization to review my child’s school records.
PERSONAL AFFECTS: Parent(s) should ensure that all personal belongings are well marked with the camper’s name. All valuable belongings such as jewelry, chains, bracelets, rings, etc., should be left at home. Many are safety hazards. Cornerstone and its staff members are not responsible for the loss or theft of any personal articles including but not limited to; jackets, books, cell-phones, mp3s, IPods, book-bags, shoes, etc. Cornerstone staff members reserve the right to search and/or confiscate any cell phone, IPod, IPads, mp3 players, computers, or any other type of digital device should Cornerstone suspect inappropriate material, content, pictures that are on the device or if the device becomes a distraction to others. In addition, Cornerstone reserves the right to search and/or confiscate any book bags, purses, notebooks, or any other type of storage container. Cornerstone staff members are the sole party to deem material/content is inappropriate.
Media Release: I acknowledge and understand that publicity activities such as interviews, photos, and videotaping may occur. I consent and permit my child, as a participant in the Kids Hope Alliance Program and events, to be photographed, videotaped, and/or interviewed for publicity activities.
MEDICATION: Cornerstone of Jacksonville will not assume the responsibility of issuing medications to any child/youth. If it is deemed that we are incapable of providing routine, necessary care due to mental or physical incapacity program participation may be denied.
NOTICE - FERPA: The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of child education records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education. Cornerstone of Jacksonville reserves the right to disclose, without consent, "directory" information such as a child’s name, address, telephone number, date and place of birth, honors and awards, and dates of attendance. Parents may request that Cornerstone of Jacksonville not disclose directory information about a child. Cornerstone must be notified in writing if a parent does not want their child’s directory information disclosed.
NOTICE - HIPAA PRIVACY: Cornerstone of Jacksonville is not required to comply with the HIPAA Privacy Rule because Cornerstone maintains health information only in children’s health records that are “education records” under FERPA and, thus, not “protected health information” under HIPAA. Because the children’s health information in education records is protected by FERPA, the HIPAA Privacy Rule excludes such information from its coverage. See the exception at paragraph (2)(i) to the definition of “protected health information” in the HIPAA Privacy Rule at 45 CFR § 160.103. Cornerstone complies with FERPA’s privacy requirements with respect to its education records.

PARTICIPATION: Cornerstone of Jacksonville reserves the right to expel any child from the summer program offered if he/she disrupts/disturbs the program’s activities and/or the program’s overall environment as determined by Cornerstone’s Staff.
POWER OF ATTORNEY: I, the undersigned am the parent/legal guardian of, the child listed above, and hereby authorize a representative of Cornerstone of Jacksonville, Inc. (DBA: Camp Discovery) to take my child to the nearest hospital for treatment, in the event that we cannot be reached in an emergency. I further authorize the hospital or a licensed Doctor of Medicine to administer treatment in case of an emergency. It is understood that I/we will be contacted by telephone, if possible, prior to treatment. By signing this Power of Attorney I release Cornerstone of Jacksonville, Inc. (DBA: Camp Discovery), the Kids Hope Alliance, and the City of Jacksonville, and/or any of their agents and/or affiliates, from any and all liability that may occur out of the child’s attendance and/or participation in the Summer Camp or Summer School Program. Furthermore, in the event of an emergency I understand that I are solely responsible for any/all expenses associated with any medical treatment received by the child.
WAIVER AND RELEASE / HOLD HARMLESS: Cornerstone of Jacksonville, Inc. (DBA Cornerstone Christian School of Jacksonville) (DBA: Camp Discovery) strives to provide a safe, clean, and caring environment for all children. However, from time to time accidents happen, including but not limited to; camper’s breaking limbs, slips and falls, cuts, bruises, etc. By signing this Agreement, you agree and understand that Cornerstone of Jacksonville, Inc. (DBA Cornerstone Christian School of Jacksonville) (DBA: Camp Discovery), the Kids Hope Alliance, and the City of Jacksonville, and their agents, staff members, teachers, board members, shareholders, camp employees, bus drivers, and/or any affiliates cannot be held responsible or liable for any injuries. You also understand and agree that it is your responsible for all out of pocket medical bills, ambulance charges, co-pays, and any other expenses regarding your child and/or family member(s). If your insurance company denies or attempts to collect expenses paid out, it is your responsibility to pay these costs. Furthermore, Cornerstone of Jacksonville, Inc. (DBA Cornerstone Christian School of Jacksonville) (DBA: Camp Discovery), shall not be held responsible for any actions or injuries caused by another child. This includes summer camp, field trips, bus transportation, van transportation, recess, extended care, as well as all summer activities.
In consideration of the acceptance of my application for the program stated, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages or which may hereafter occur to me as a result of participation in said event. This release is intended to discharge in advance Cornerstone of Jacksonville, Inc. (DBA: Camp Discovery), Cornerstone Christian School, Inc., the Kids Hope Alliance, and the City of Jacksonville, their agents, officials, officers, employees, shareholders, board members, volunteers and agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assignees. This waiver and release is intended to and does release the mentioned parties from any and all liability for damages or injuries on account of or in any way related to or growing out of my negligence, my child’s negligence, the negligence of another child, an employee’s negligence and/or the negligence of third parties, including but not limited to negligence in the construction, maintenance and upkeep of the facility and its equipment, negligence in training or negligence in supervision in addition to but not limited to negligence of Cornerstone of Jacksonville, Inc. (DBA: Camp Discovery) and/or Cornerstone Christian School, Inc. and/or their agents, related companies, servants, employees, insurers, successors and assigns. In the event of an emergency if a parents/guardian cannot be reached, I hereby give my permission for Cornerstone to authorize emergency medical treatment for my child. I understand that I will be 100% responsible for any/all expenses associated with any medical treatment.
SIGNATURE: By signing this application, you are acknowledging and agreeing to the terms and conditions set forth within this Summer Camp Program Agreement.
